Sealed With A Kiss……

For several years I got Juvederm injections to plump my thinning lips.  I loved the look but unfortunately, it only lasted 3-4 months for me, so the expense mounted with the frequency of the fillers.  When Dr. Sternberg told me about lip implants I jumped at the chance to have them inserted!  No more injections and every morning I look in the mirror and think, “oh, it looks like I just had my lips injected.”  It is a very natural look and no one knows I have the implants until I mention it.

It can be done under local anesthesia, like you get when you go to the dentist.  Mine were done during a surgical procedure I had planned.  I knew someone who had it done weeks before under local anesthesia, so I got to see her lips and knew what to expect.  She said she could not drink through a straw, as she was told to do (per post procedure guidelines), but could only eat with a baby spoon for several days.  I found I could not use a spoon, but could better use a straw for the first several days. I also applied ice to the lips which felt good and helped with the slight swelling.  I returned to the office a week later with very little swelling noticeable and had 2 small stitches removed from the corner of my mouth.

Quit Seeing Double on Your Chin

Some people really have double chins — Chris Christie, Jobba the Hut, and Orson Welles, to name a few. But truth be told, the vast majority of us will have some sort of junk going on under our chin as we age.

The clinical term is submental fullness. If you don’t want to say “double chin” say submental fullness. Considering pretty much everyone has some degree of a double chin at some point, it’s odd that more attention hasn’t been given the problem by the aesthetic industry.

Now that has changed with Kybella®, a prescription medicine available at Ponte Vedra Plastic Surgery. Kybella® is the brand name for a prescription injection formulated with deoxycholic acid to improve the appearance and profile of moderate to severe fat below the chin, commonly known as a double chin.

How does a facelift work?

Q: What is a good anti-aging product regimen?

A: Topical key elements for healthy skin are as an important regimen as your daily oral dose of vitamins. Everyone should be using a medical grade foundation of topical products beginning with a good cleanser, toner, topical vitamin A, C, antioxidants, peptides, growth factors and sunscreen.

Q: What is the average topically applied product penetration rate?

A: The approximate amount is in the region of only 7-10%.

Q: How can I improve the product penetration rate?

A: Our skin has a layer of dead cells piling up as a barrier to the world. Getting through this barrier and exfoliation the dead cells off is a key to increasing the penetration rate of products. So what does the Halo do?

Wondering what to expect following the treatment?

This really is the best part about using Halo.  The procedure is done with topical local numbing cream.  There is minimal down-time.  You can expect to have red, slightly swollen facial skin for three to four days.  This is a great procedure to have done in the office on Thursday and go back to work on Monday.  There is no peeling.  The skin will feel like sand paper for a few days and there will be small dark flakes that you will notice on your skin for a few days afterwards.  These dark flakes are the underlying pigmentation that is being extruded.

How does Halo work?

The Halo is a hybrid laser that has a partial ablative and non-ablative property.  This allows minimal downtime with maximal effects.  The procedure requires three treatments 6 to 8 weeks apart and once per year thereafter.  There is exciting literature that has demonstrated significant long term youthful effects of this combined treatment.  See below:

What is a Liquid FaceLift?

Have you heard the term “liquid lift, or liquid facelift” when searching the internet or reading a magazine? If you have, and wondered what that means, then the following article will be very interesting and informative for you.

Q:  What is the “liquid face lift?”

A:   This is a term used when facial cosmetics is improved using only fillers and other non-surgical modalities. For instance, a patient may have fillers injected into the cheeks, eyelids, oral areas, and temples, while having Botox injected into the forehead, brows, eyes, and neck. This accomplishes a very nice, sometimes dramatic, improvement without having to undergo surgery.

Q:   Am I a candidate for the “liquid lift?”

A:   The best candidates for a liquid lift are patients with early to moderate facial aging who are not quite ready for eyelid lift, facelift, or midface lifts. This would be best determined during a consultation. We usually assess the patient, their goals, downtime limits, degree of facial aging, and whether the procedure is financially better than a surgical approach. Patients in their 40’s and early 50’s are usually the best suited for this treatment. They will have a very nice, restoration of youthfulness without looking surgically altered and requiring no time away from work.

Q:   What injections are commonly used for this?

A:    We usually determine the best filler to use based on areas we want to improve. A fine filler like Restylane© would be used for the lower lid creases, and smile lines around the mouth. A middle volume filler like Juvaderm© is best for the lips, corner of the mouth, chin, and nasolabial folds. Heavy lifters, like Voluma©, and Radiesse©, are best used to augment the cheeks, temples, lateral brows, and deformities of the nose.  Finally, Sculptra©, is used to thicken the skin and add more fullness to the entire face. In conjunction with these, we often used Dysport or Botox to soften the appearance of the brow furrows, lift the eyebrows, relax the squint lines around the eyes, and even to relax the lines around the lips and in the neck.  Again, the decision about what products to use is left up to a full discussion between you and your surgeon or nurse practitioner.

Q:   What is the downtime I should expect?

A:    Downtime is usually only about 24-48 hours. During this time you will have some swelling, and mild bruising. You can shower and use makeup right away. We recommend using ice and elevating your head on pillows the day of the procedure. You can go back to work the next day if you feel like you are ready. You may go back to working out after 48 hours.

Q:   How long does this type of treatment last?

A:   Depending on the types of fillers used (each has a different length of action), you should expect to get between 6-9 months from one treatment, although many patients report high satisfaction at one year. Botox© and Dysport© usually last 3-4 months. This treatment is best for patients who want to look their best during an event or Holiday season for example.

Q    Can I combine this with any other procedures?

A:   Yes, a liquid lift can be combined with other procedures to improve the health of the skin and look of the face. This is commonly combined with medical facial treatments, and medical grade skin care to improve the skin texture, tone, and color.  Also, we use skin tightening lasers (read about HALO and BBL) to improve the skin texture and sun damage. Together, facial volumization and advanced skin treatment, results in a dramatic improvement in facial appearance and youthfulness.

Exfoliate; Spring Out of the Old and In With Your New Radiant Skin

ExfoliationOn average your skin can shed 30,000 to 40,000 dead cells per minute. Exfoliation is very important when rejuvenating weary winter skin. Exposure to the “dry” indoor heat during the winter months can drain moisture from the skin and add layers of dead skin cells, leaving your skin feeling heavy and looking unhealthy. In Search of the Holy Grail

Weight ScaleFor decades there has been a relentless search for effortless weight loss and painless cosmetic surgery. We might call these goals the “holy grail” of plastic surgery. Last year Americans spent over $11 billion on surgical and non-surgical cosmetic procedures and the potential market is likely much, much larger. The only thing that holds many of us back is the effort, pain and recovery typically required to improve/reshape our bodies. Without a doubt, there are tremendous profits to be made by anyone that can produce consistent results with minimal effort, pain and downtime. And wherever there are big profits to be made there will invariably be snake-oil salesmen trying to get a piece of the action. I cannot stress too strongly that it is a “buyer beware” market out there. Remember what your mother always said: “If it sounds too good to be true…..it probably is.”

With that warning out of the way, let’s look at where the search stands today.

I think the hottest area of research right now is the search for a non-invasive technique or technology that will remove fat without the need for surgery or anesthesia. There are several companies currently touting such technologies…and ALL are fiercely competing for our hard-earned dollars with big promises and multi-million dollar marketing campaigns. Let’s take a look at a few of them one at a time:

1. Cold lasers: This technology, most prominently marketed as Zerona and Lipo Light, uses non-invasive low level laser therapy(LLLT), or “cold laser” therapy, to purportedly disrupt the cell membranes of fat cells, essentially deflating them. Your body then eliminates the fat cell contents. Zerona has been modestly successful marketing their laser technology in conjunction with diet and exercise programs making it difficult to determine the true cause of any positive results.

2. Cryolipolysis: This technology, most widely marketed as Coolsculpting by Zeltiq, uses targeted cooling of the fat tissue, sparing injury to the overlying skin, in an attempt to kill fat cells. This technology, like Zerona, has been FDA approved for fat reduction, but experience to date is mixed. Our understanding is that patient satisfaction rates are in the 40-50% range. At $1000-3000/treatment, the challenge will be how to deal with the 50-60% of patients that are unhappy!

3. Ultrasound: Ultrasonic energy has been used for decades in medicine to treat various maladies such as solid tumors and kidney stones. Now it is being promoted as a way to break up fat cells without injury to the skin. The most promising and exciting technology is a type of focused ultrasonic energy called HIFU, or High Intensity Focused Ultrasound. Early studies seem to indicate modest success in carefully selected patients. While we have not yet committed to this technology, we believe it is exciting and are anxious to see results from second and third generation products.

4. Radiofrequency: While not marketed specifically for fat, I include this technology here because of its prevalence in the aesthetic marketplace. Most popularly known as Thermage, this technology is promoted as a way to tighten loose skin by transferring energy via radiofrequency waves to the deep layers of the skin. As with the other technologies listed above, results to date have been modest at best. I have seen many disappointed patients that have spent thousands on this technology only to end up in our office for a facelift.

So where does this leave us? As disappointing as it sounds, it’s not all bad. Future generations of one or more of the technologies listed above may prove to be more promising. Currently, there is no shortage of research going on with all of these and more. Unfortunately the holy grail, though perhaps on the horizon, continues to elude us. So for now I must still warn: “Buyer Beware”.
